About Us: Join Hertfordshire County Council's 0-25 Together Service, a dynamic Disability Service committed to supporting individuals aged 0 to 25. We are currently recruiting for a dedicated Social Worker to join our team in Apsley and Stevenage. This role offers a unique opportunity to make a positive impact on the lives of individuals with disabilities and their families.
Key Responsibilities:
- Conduct assessments and develop care plans for individuals aged 0 to 25 with disabilities.
- Work collaboratively with multi-disciplinary teams to ensure holistic and person-centered support.
- Provide ongoing support, advocacy, and intervention to enhance the well-being of service users.
- Collaborate with external agencies and professionals to coordinate effective service delivery.
- Maintain accurate and up-to-date records, adhering to statutory requirements.
- Contribute to safeguarding practices and respond to safeguarding concerns appropriately.
- Engage in continuous professional development to stay abreast of relevant legislation and best practices.
Skills and Experience Requirements:
- Full UK Driving Licence: Essential for work-related travel.
- 1 year post-qualifying experience and ASYE Completion:
- Criteria depends on the quality of relevant experience gained for the applied team.
- Social Worker Qualification (England & Wales): Recognized qualification in social work.
- Social Work England Registration (previously called HCPC): Current and valid registration.
