Job Requirements/Description

We are seeking an experienced Clinical Pharmacist to lead and develop a patient-centered clinical pharmacist service across our Regional Recruitment network. As a vital member of a multi-disciplinary healthcare team, you will be actively involved in patient care, particularly in managing chronic diseases and conducting in-depth clinical medication reviews. This role focuses on improving healthcare quality, patient outcomes, and operational efficiency within general practice settings.

Key Responsibilities

Patient-Facing Clinical Duties:

- Provide direct patient care in a primary care setting, specifically managing chronic disease areas within local practices.
- Conduct comprehensive clinical medication reviews, especially for patients with complex medication regimens (polypharmacy), elderly patients, individuals in residential care, and those with multiple co-morbidities.
- Engage in proactive case management for at-risk patients, ensuring optimized and safe medication use.

Primary Support for General Practice:

- Serve as the primary point of contact for prescription and medication queries from general practice staff, addressing both clinical and administrative issues.
- Support the repeat prescription system to ensure timely and accurate medication dispensing.
- Manage acute prescription requests and perform medicines reconciliation during patient transitions of care.
- Implement and monitor systems that promote safer prescribing practices, including regular audits and process improvements.

Clinical Leadership and Medicines Optimisation:

- Act as a clinical leader on medicines optimisation, promoting best practices and driving initiatives for quality improvement.
- Provide expert guidance on clinical medicines, offering support to healthcare teams on complex cases, and supporting holistic patient care.
- Manage aspects of the Quality and Outcomes Framework (QOF) and enhanced services within the PCN, contributing to regional healthcare targets.

Integration and Collaboration:

- Strengthen links with community and hospital pharmacies to enhance skill mix and improve continuity of care for patients.
- Facilitate collaboration across the multidisciplinary team, including GPs, nurses, and other allied healthcare professionals, to ensure integrated, high-quality care delivery.
- Engage in community outreach, when necessary, to support public health initiatives and improve healthcare accessibility for underserved populations.

Operational Efficiency and Patient Outcomes:

- Work proactively to improve healthcare efficiency within practices, supporting service delivery models that enhance patient care and reduce workload strain.
- Participate in continuous quality improvement initiatives, focusing on patient outcomes and reducing risks associated with medication use.
- Lead on initiatives to reduce healthcare disparities within the community, especially around medication access and education.

Required Qualifications and Experience:

- Registration: Mandatory registration with the General Pharmaceutical Council (GPhC).
- Professional Membership: Active membership with the Royal Pharmaceutical Society, with a preference for candidates who are members or working towards Faculty membership.
- Educational Background: A Master s degree in Pharmacy (MPharm) or an equivalent qualification.
- Advanced Training: Specialist knowledge gained through a postgraduate diploma or equivalent level training/experience in clinical pharmacy.
- Prescribing Authority: Independent prescribing qualification or a commitment to achieve this qualification within a specified timeframe.

Skills and Competencies:

- Clinical Expertise: Strong clinical knowledge and practical experience in chronic disease management, medication safety, and medicines optimisation.
- Communication Skills: Excellent interpersonal skills with the ability to communicate complex information to patients and healthcare staff effectively.
- Organisational Skills: Ability to manage workload efficiently, prioritising tasks and making sound clinical decisions in a busy healthcare environment.
- Teamwork and Leadership: A collaborative team player with the capability to lead clinical improvement initiatives and support colleagues across a multi-disciplinary team.
- Adaptability: Flexibility to work in diverse healthcare settings and adapt to changing priorities as part of the Regional Recruitment PCN network.
