• Qualified Social Worker / Social Work Professional required to join a statutory Adults Service
• Office base: Surrey - Redhill
• Up to 37ph
• Hybrid working
• You must have a social work qualification and full SWE registration as well as relevant experience to be considered

Responsibilities:

• Conducting assessments of individuals' needs for social care and support, considering

physical, emotional, and environmental factors.

• Developing and implementing care plans tailored to everyone's circumstances, promoting

independence and well-being.

• Providing advice, guidance, and practical support to individuals and their families/carers

regarding available services and resources.

• Liaising with healthcare professionals, community agencies, and other stakeholders to

coordinate and deliver integrated care services.

• Safeguarding vulnerable adults by identifying and responding to instances of abuse, neglect,

or exploitation, following established protocols and procedures.

• Advocating for clients' rights and interests within the social care system, ensuring they

receive appropriate support and access to necessary resources.

• Maintaining accurate and up-to-date records of assessments, interventions, and outcomes in accordance with regulatory standards and organizational policies.
• Participating in multidisciplinary team meetings, case conferences, and training sessions to

enhance professional development and service deliver

To be considered, it is essential that you have a recognised Social Work qualification (Social Work Degree or Diploma in Social Work) and full Social Work England registration as well as recent or significant relevant experience in a similar role.
