Role Description
This is a full-time, on-site role for a highly experienced and qualified Care Manager baser at Caremark Hull & East Riding in Hessle, Hull. The Care Manager will be responsible for overseeing case management, leading best practice in compliance, coordinating care services and implementing social services and medicine-related support for individuals in need.
