Young People's Disability Project Worker-Permanent.

We have an exciting opportunity for a number of positions as a Young People's Disability Project Worker within the Children, Families and Education's - 0-25 SEND service.

(Grade 4-18 hours per week. Term Time only)

We are seeking to recruit skilled and experienced workers who have the knowledge and experience to provide specific support to the Youth Disability Project team, including dealing with vulnerable children, young people and their families within the Children's, Families and Education Service.

Specifically we are looking for a worker who is able to cover our Post 19 Pathways to Adulthood Programme. Working with young adults with SEND.

This will be a demanding but equally rewarding position, in not only providing direct support to young people, but also building on and developing our approach. The post will help bridge the gap between special education and care in the community. Leading to much greater engagement with social workers, the Transitions team, community-based partners, along with linking up with adult services both within (Active Lives) and outside of the council.

The role:

The post holder will deliver high quality programmes of outcome-based interventions that are effective for young people with disabilities, autism and additional needs and their communities. Working under direction and own initiative.

Deliver targeted and specialist educational programmes through creative and recreational based interventions and life skills programmes that have healthy personal, independent and social development outcomes at their core.

Work with Young People, listening to their views and creating a person-centred learning plan that supports the individual to develop life skills and become independent.

To take a role in keeping young people safe whilst they are on site at Youth Disability Service/ Post 19 Pathways programme and/or attending an off-site Youth Disability Service/Post 19-Pathways activity.

Ability to work up to 3 days per week.

You must have:

  • Excellent engagement skills with children and young people with SEND, and their families
  • Able to communicate with young people with range of communication techniques like signing / Makaton, making and using visual resources.
  • Creative approach to direct delivery
  • Able to use initiative to further links/development Able to deliver high quality targeted interventions
  • Able to develop and maintain relationships with young people who have complex needs
  • Able to use IT to document plans & outcomes. To measure and share information.
  • Able to maintain outcomes based quality assurance
  • To be self-motivated
  • Ability to manage conflict with a solution focused approach
